How to Apply
Notifications of grant decisions are usually sent in mid-December.
To apply for a grant, download an application
form, which you can then fill out on screen, print,
and mail. To receive the form by mail, please
contact us.
Please include with your application:
- A list of your committee or Board of Directors
- Your most recent financial statements - audited, if available
- A current operating budget
- Your application is reviewed by a committee, so please make sure that it is legible and easy to copy. Faxed and e-mailed applications will not be accepted.
- The committee receives only the formal application, so please do not send additional attachments.
